Student Athletic Paperwork

CCSD-sponsored middle school sports include Flag Football and Cross-Country Running in the fall, Basketball in the Winter, and Soccer in the Spring. All sports are for both boys and girls. Additionally, we have Cheerleading during the basketball season. If you plan to have your student participate in any or all of these, we urge you to register your student on Aktivate at registermyathlete.com as soon as possible.

Flag Football begins the second week of school. In addition, you will need to have your student get a physical and have the doctor fill out the necessary forms. Students must have had their physicals BEFORE they can try out for any sport. Make your appointments for physicals as soon as possible, and don't wait until the season of the sport. Many students could not play or even try out because they had to wait for a doctor's appointment after tryout dates.
We suggest choosing all four sports. Doing so does not obligate participation, making registration easier if playing more than one sport.
